Activities Consent Form

Please read the After School Activities Policy before downloading the After-School Activities Consent Form, which is available here in pdf. This signed form should be returned to the green post box along with payment for the course. Note that places in each group are limited and a pupil may be excluded from an activity if their behaviour is in breach of policy.

In case we need to reach you during the After-School Activity hour(s) please ensure that you are accessible at the number you provide in the form.

In addition, the committee would like to stress that the After-School Activity tutors are not responsible for children after the end of class. If you are running late for collecting your child, it may be advisable to have an arrangement with another parent/guardian who can supervise.

Paper copies of the After School Activities Policy are available from the committee on request.
